Calderdale Council, are pleased to offer several opportunities to undertake the role of Independent Living Officer (ILO) within the Reablement Team.
The role has been redesigned and regraded to encourage people who are thinking about a career in Health and Social Care, combining elements of Occupational Therapy, modern approaches to community rehabilitation, reablement and the innovative use of Assistive Technology to support impendence and community inclusion.
These roles have been regraded to reflect the new approaches we are developing and can be the first step in a rewarding career, covering a variety of professional roles in health and local government. The reablement team consists of a range of health and social care professionals and supports a Home First pathway and aims to:
• Provide reablement to people in their own homes - including assessment and provision of equipment, aids and adaptations, assistive technology.
• Reduce preventable admissions to hospital.
• Reduce length of stays in hospital by supporting safe and timely discharges using a Home First model.
• Provide a person-centred approach to optimise independence and confidence, enable recovery and prevent a decline in functional ability.
• Undertake assessment for longer term care for those requiring on-going support.
The role of ILO will be to support people to live as independently as possible through engagement, assessment and provision of equipment and aids to support independent living. You will assess, provide and install aids, equipment, adaptations, personal care, medicines management, rehabilitation exercises and support for carers to meet identified needs to support people to live safely at home and maximise independence.
