About the role...
An exciting opportunity for a Recovery Coordinator has arisen within our HOPE team.
The HOPE team are the first point of contact in hospitals, GP surgeries and other community settings in County Durham. We will assess the client and refer to the appropriate team within the service. We also work collaboratively with service trainers to deliver education and awareness, raising activities for stakeholders and communities, as well as providing holistic and structured recovery coordination and psychosocial interventions to service clients, their families and carers.
The role of a Hope Recovery Coordinator is to manage a service user caseload, devising, implementing, and reviewing ongoing recovery plans and treatment, liaising closely with the full range of workers and skills within the service team, including family members and significant others as appropriate. You will help set goals in recovery and help the service user achieve their goals in a safe and supportive way.
The Hope Recovery Co-ordination is a diverse and rewarding role, Key Duties:
Working across East Durham, you will be required to work as part of a team to provide recovery planning and case management to substance misuse clients (Drug and Alcohol) at different stages of their recovery journey, from the assessment and engagement phase providing holistic and structured intervention and psychosocial interventions.
Please note that this role includes weekend and late night shifts (on a rolling rota) and a willingness to travel across Durham is essential.

- Facilitate and promote individuals' progression from initial engagement, through structured treatment, to self-sustained recovery.
- Reduce substance-related harm to individuals and the wider community.
- Working collaboratively with key partners and colleagues to achieve the best health and well-being outcomes for individuals.
- Work within the community in GP surgeries, Mental health services and other partner agencies
