The Vacancy This role is based within Platfform's Supported Housing scheme within the Vale of Glamorgan. The scheme is provides tenancy related support to persons with complex and/or enduring mental health needs as evident either through their support history, because they are under the care of secondary mental health services, or both. The successful candidate will provide a range of social, emotional and practical support to people with mental health and associated problems impacting their ability to maintain a stable tenancy, through semi-independent living. The support will be expected to be within commissioner's guidelines i.e. the Supporting People Framework, and in line with Platfform's value base. The role holder will be expected to provide support and mentoring to Support Workers in the development of individual support plans (ISPs) and risk assessments (RAs), as well as take lead during crisis support. Compliance with and maintenance of all necessary administrative and reporting systems will be required. The Vacancy Caerphilly supported housing consists of three homes, Ty Oborne, Ty Short and Jubilee House, each home offers a different level of support alongside Star flats which offers people targeted support delivered by a floating support team who work with people who are on the waiting list for one of the homes or moving on to their own accommodation. Independent Reviewing Officer - Isle of Wight Role: IRO Location: Isle of Wight Pay: up to £45/hr Contract: Full Time (Locum) Our Local Authority client on the Isle of Wight is currently looking for a IRO to join their team in Children's Services. The role: To co-ordinate and chair looked after children's reviews within statutory timescales placing the child at the centre of the reviewing process ensuring their thoughts and feelings are considered in the planning process. To review and monitor the effectiveness of care plans for looked after children. To ensure that Care plans are promptly implemented, that agreed actions are being progressed and that there is no drift in permanency planning. Key Responsibilities: To monitor the Local Authority's review of looked after children's care plans, ensuring that actions required to implement the care plan are progressed and carried out in a timely manner and that outcomes are monitored. To endorse care plans, make and record recommendations and actions that are required to meet the reviewed child's needs, achieving timely outcomes. To make recommendations about discontinuing the child protection plans for children subject to such plans who become looked after. To refer to relevant Senior Managers cases and practice that are not addressing the care plan and to implement guidance and procedure for the referral to CAFCASS where the criteria for doing so is met. To meet with children to ascertain their wishes and feelings and ensure that this are considered within the planning and review process.
