Homelessness Support Coordinator Application Deadline: 9 November 2025 Department: Resettlement - Penzance Employment Type: Permanent - Full Time Location: Penzance Reporting To: Faye Hookins Compensation: GBP 29,352 / year Description As a Homelessness Support Coordinator, you will work with individuals who have been rough sleeping or who are at risk of rough sleeping in Cornwall. You will carry out intensive assessments to identify clients' support and housing needs, advocate for them and help them to access the services they need. As part of the Non-Residential team primarily based at the Breadline Centre in Penzance, you will also be expected to provide cover at the Truro Resource Centre when required. Your duties will include the organisation of groups and meetings within the projects, supporting colleagues with assessments and the reconnection of clients to their hometowns, and facilitating the "drop in" sessions provided by the organisation. Whilst you will normally work within the services' opening hours, there will be a requirement for occasional evening and weekend work as directed by the Non-Residential Service Manager. This will include providing support to St Petrocs' emergency "Winter Service" during the colder months. Interviews are expected to take place on Thursday 13th November. What you'll be doing. <ol><li>Work with clients to provide an intensive assessment of their social, welfare and housing needs.</li><li>Prepare a comprehensive risk management and support plan with the client to address their needs.</li><li>Locate and provide housing, social and welfare solutions in accordance with the needs assessment.</li><li>Liaise with partner and specialist agencies to enable rough sleepers to access their services (e.g. housing, drug and alcohol, health, welfare rights and legal services).</li><li>Establish a profile of the needs of service users and maintain digital records. This will include keeping accurate file and case notes.</li><li>Negotiate access for clients to any appropriate forms of accommodation and ensure that appropriate support is available.</li><li>Advocate for people sleeping rough in accessing services, including 'reconciliation' work with other agencies.</li><li>Liaise regularly and work closely with partner, voluntary and statutory agencies. Weekend Housing Support Worker - Young People's Services

Job summary An exciting opportunity has arisen for a Practice Nurse to work with the Cornwall Health for Homeless team. Cornwall Health for Homeless is a GP Practice &amp; we aim to provide a range of healthcare services to people without homes in Cornwall. The service runs regular clinics in Truro, Camborne and Penzance that are open to people who are either street homeless or in temporary accommodation. To achieve this, we work with other organisations which include St Petroc's and Coastline Homeless Service as well as other local and national statutory groups. Part of our role is to help people live more stable lives and gain access to suitable accommodation. Over the last two years we have expanded our service from our three clinic sites to include outreach clinics countywide and we are seeking a Practice Nurse (Band 6) to join the team in an extended role, supporting &amp; working collaboratively with the GPs. This is a full-time post, working agreed hours Monday to Friday, with the possibility of doing outreach work some of the time. Compassion are flexibility are key to this varied role. Main duties of the job To provide primary care tailored to people experiencing homelessness, including health assessments, chronic disease management, vaccinations, wound care, and sexual/reproductive health services. Deliver health education, screening for infectious diseases, and support lifestyle and wellbeing advice. Engage patients flexibly, building trust with those who may face barriers to care, irregular attendance, or trauma histories. Work closely with multidisciplinary teams, outreach services, and social care to coordinate care, advocate for patients, and ensure continuity, aiming to reduce health inequalities and improve overall health outcomes.
