When you choose to join us, you're choosing more than just a job.... you're choosing to make a difference" We have an exciting opportunity for 3x Recovery Coordinators to join County Durham Drug and Alcohol Recovery Service as a Recovery Coordinator working in The centre for change, based in either Horden or Peterlee. The County Durham service has recently been rated 'Outstanding' by the CQC and is a great place to work. The service supports adults across County Durham with drug and alcohol issues. The successful post holder will match our organisational ambition and commitment to provide the highest standards of service delivery across County Durham, engaging with local communities and supporting their priorities. Key Duties:<ul><li>Manage an adult caseload of clients with drug and alcohol issues, reviewing on-going care and treatment and liaising closely with a full range of workers and skills within the multi-agency team, including family members and significant others.</li><li>Devising, implementing, and reviewing ongoing recovery and treatment plans.</li><li>Facilitate and promote individuals' progression from initial engagement, through structured treatment, to self-sustained recovery.</li><li>Reduce drug or alcohol-related harm to individuals and the wider community.</li><li>Working collaboratively with key partners and colleagues to achieve the best health and well-being outcomes for individuals.</li><li>Support with evening and weekend clinics when needed.</li></ul>To view the full job description please click on the link. As a Recovery Coordinator, you will provide 1:1 support for service users, from brief interventions through to structured treatment, including groups, to ensure that every recovery journey is tailored to individual priorities. You will take a "whole person" holistic approach to ensure that every service user's package of care encompasses all areas of their wellbeing, to achieve the best possible outcomes for every person. <ul><li>Manage a service user caseload, devising, implementing and reviewing on-going recovery plans and treatment, liaising closely with the full range of workers and skills within the service team, including family members and significant others as appropriate.</li><li>Carry out triage/screening assessments and comprehensive assessments with service users.</li><li>Work collaboratively with partner agencies to undertake shared assessments, key working and interventions to promote individual service user progression from engagement, through structured treatment to self-sustained recovery.</li><li>Facilitate service user access to treatment and community resources to increase recovery and social capital, supporting service users and their families within their local community To lead effective case management for allocated service users, including on-going processes such as risk assessments, risk management plans and safeguarding assessments and plans.
