Homeless Support Worker

The Strategic Housing Team are seeking an experienced and knowledgeable individual to join the Homeless Relief Team as a Senior Homeless Relief Officer. This position is for 1 permanent full time officer, working 37 hours per week over a Monday - Friday working pattern. The team are currently working on a hybrid basis, 50/50 split between the office and remote working. The team work from both the Macclesfield and Crewe offices, the ability to travel within the borough is essential. About the role To be the first point of contact within the Homelessness Prevention, Advice and Resettlement Team for customers and partner agencies. Housing and Homelessness - Hospital Discharge Housing Coordinator

Job summary An exciting opportunity has arisen to join an existing UHD multidisciplinary team, working on the Poole and Bournemouth sites, but also undertaking outreach to review patients in the community. The aim of the service is to care for patients accessing UHD services who are homeless or at risk of homelessness and who may also have mental health, alcohol and drug misuse issues. The role will also involve the care of patients undergoing treatment for Chronic Hepatitis C across Dorset as part of the Dorset Hepatitis C Elimination Programme. Solihull Integrated Addiction Service (SIAS) is a partnership between four organisations jointly responsible for the delivery of the drug, alcohol, homelessness and gambling services, in the Borough of Solihull. We offer services for adults who use or are affected by substance use, homelessness and gambling. The housing service covers support for clients who are homeless and at risk of becoming homeless, as well as a more structured longer term floating support service to enable clients to maintain their tenancies. About The Role The Support Worker will provide a structured, personalised resettlement service to every service user at Mildmay House. To seek to support service users who have experienced homelessness to move into their own homes in a planned and supported way. To offer advice, training, information and one-to-one/group support to help service users through this process. The support offered very much depends on the individual, but can include advice on housing rights, accompanying individuals to meetings, and practical support to move in, such as obtaining furniture and setting up payment plans for household bills. Ensure that service users who are ready to move on are given the security and support to maximise their independence, dignity and choice, in accordance with the aims and objectives of the project. Please note this is a full time role working on a rota basis - generally a week of day shifts, a week of night shifts and working every other weekend. This includes providing cover within a 24-hour service on a fixed rota including working evenings, weekends, and sleep overs (on site). The Homeless Accommodation Service directly manages and provides housing related support to single people and families experiencing homelessness / rough sleeping, across 50 units of emergency accommodation in Carlisle, some of which is staffed 24 hours a day on fixed rotas (this post is one of those roles).
