The role As a Duty Practitioner you'll provide support and advice to people who have experienced homelessness at our male or female scheme. You'll make sure customers feel comfortable and can manage their tenancy, as well as signposting them to other support such as housing, health and welfare. This role is 37.5 hours on a rota basis, covering 8am - 11pm, Monday to Sunday. Job requirements <ul> <li>Provide and maintain a friendly and supportive environment at the scheme.</li> <li>Support customers to manage their rent and service charge payments, including their benefit claims where necessary.</li> <li>Complete detailed sign-ups to make sure new customers understand their rights and responsibilities. Homelessness Support Worker

Job summary This is an exciting opportunity for an experienced Mental Health Practitioner to work within the Nottingham City Homeless Mental Health Team. The role involves outreach work to reach those experiencing homelessness to provide mental health support. It is a great time to be joining the team as we are starting a Health Innovation project and the successful candidate will have the opportunity to contribute to service development. The team will be working closely with the Alcohol Long Term Conditions Team, Drug and Alcohol Liaison Teams and Physical Health Care Team to provide wrap around care to people experiencing homelessness in the county. We will provide the successful candidate with the opportunity to develop skills needed to work within a community setting. The training offers will include specific skills required for working in the homeless sector, for example, trauma informed care, substance use and housing knowledge. We are looking for someone experienced in mental health and keen to learn, with an interest in working with people with Severe and Multiple Disadvantage (SMD). We do not have a set base so the location in the county this can be negotiated with the successful candidate. Main duties of the job The post holder will be expected to hold a caseload and work with service users and their families to support personalised care plans and provide specialist interventions. Job summary This is an exciting role to help provide an innovative model of care for homeless people admitted to St Georges Hospital. This post is open to individuals with occupational therapy backgrounds who have an interest in homeless health issues. They will provide clinical expertise and case management for patients referred to the service. They will have an understanding of the complex tri-morbid factors that the patient cohort experience, taking this into consideration in assessment and recommendations made. Main duties of the job The post holder will be engaged in significant partnership working aimed at improving hospital discharge for individuals, and also for groups of patients where patterns emerge. This partnership working will be with NHS services, GP's, Housing departments, Social Care and Voluntary Sector organisations. The post holder will be responsible for ensuring that there are processes in place for the holistic assessment, planning and provision of interventions. The post holder will work alongside a Specialist General Practitioner a Homelessness Nurse Specialist, 2 Housing Officers, Adult Violence Practitioner and Care Navigator, to provide additional support to homeless people. The post holder will be responsible for supporting the Homeless Team in the enhanced management of homeless people on the wards. The post holder will provide direct support to the Homeless Team, medical and surgical teams, therapists and wards in discharging their patients in a safe and timely manner. The team work closely with staff across all disciplines between hospital and community to ensure a safe, timely and effective transfer of patient care on discharge. The team aims to improve the health of homeless people, provide holistic well co-ordinated care, reduce delayed discharges and also reduce premature discharges of homeless patients with the aim of reducing future unscheduled admissions and ED attendances.
