Overview Change Grow Live delivers an integrated rough sleeper support service in Newham, providing a wide range of initiatives to ensure the best possible outcomes for people experiencing rough sleeping in Newham. A fantastic, new opportunity has arisen to become an Outreach Caseworker, assertively engaging with individuals who are rough sleeping. We are looking to recruit a passionate and motivated Outreach Caseworker to the team. Working in partnership with trusted local providers, the integrated rough sleeper support service provides outreach work, social care navigation, housing first services, accommodation, move-on support, a day centre, immigration advice and education, training and employment opportunities. Location: Newham Sessional Hours: 0 Sessional Salary: £19.85 per hour * Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. Responsibilities About the role: Our ideal candidate will have the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. A lot of the role is remote working and admin work can be done from home. Each Outreach Caseworker is usually on an early shift 1-3 times per week. Street Outreach Team Worker (homelessness services)

About the Role Southend Medical Centre are looking for a motivated individual who has a passion on tackling health inequalities, and particularly an interest in healthcare for the homeless population. We are seeking an experienced prescribing paramedic or nurse on an 11 month maternity contract, commencing in September, on a part-time basis for approximately 22.5 hours a week, working over 3 days. Hours and working pattern can be negotiable for a suitable candidate. Reporting to a GP clinical lead, previous experience of working with this population group is desired but not essential. Southend Integrated Healthcare for the Homeless (SIHH) provides a service specific to homeless individuals within Southend on Sea. The role will include a range of activities addressing the individual health and wellbeing, the development of pathways into specialist services, caring for the homeless and improve the overall health inequalities by: <ul> <li>Health promotion</li> <li>General health screening</li> <li>Engagement in primary care services and facilitate access to organised health care</li> <li>Integration of the service with secondary care services, mental health services and local drug and alcohol services</li> <li>Working closely with local voluntary providers involved in looking after the local homeless population.</li></ul>The purpose of this role will be imperative in improving integrated pathways to prevent fragmented services to become efficient and effective. The role is specifically to support those to improve health outcomes. The collective aim of the service is to help those with a range of health needs including drug and alcohol to access the appropriate treatment, to provide clinical assessment and treatment in the community along with signposting clients to GP services, secondary care, drug and alcohol services.
