Homeless Recovery Worker

Job description Part Time 22.5 hours per week, Fixed Term for 18 months. Are you passionate about making a difference in the lives of young people? Bristol City Council is seeking a dynamic and dedicated professional to join our Youth Homelessness Prevention team. This strategic role is pivotal in promoting and supporting the Council's objective of improving accommodation transitions for those leaving care. This is an exciting new role, funded by a Department for Education grant to promote innovative solutions to prevent care leaver homelessness. The role will focus on developing better systems across housing and children's social care to achieve systemic change which maintains improved outcomes for care leavers. This will include creating multi-agency meetings / forums, scrutiny of data to understand our child in care population and future accommodation commissioning needs, understanding the holistic needs of children approaching 18, championing them within the accommodation pathway, creating systems to prioritise the right young people for in demand accommodation, quality assurance of practice around care leaver accommodation and homelessness, and full review and implementation of the Joint Housing Protocol for Care Leavers. You will work alongside commissioners to lead on our innovative project to house care leavers in shared supported housing, matching young people to the accommodation and holding risk management / provider meetings as required. The role will be managed within the Youth Homelessness Prevention Project team, working across both City Hall and B-Bond offices, to achieve the balance between strategic and operational work that the post requires. We are looking for someone who can successfully bring stakeholders together to improve accommodation transitions for young people leaving care. You will be able to confidently act as a champion for best outcomes for care leavers in relation to accommodation and will be able to lead challenging negotiations with a range of partners across differing priorities, including developing and delivering training and briefing sessions across a multi-agency workforce. You will have well developed skills in data analysis (including use of large spreadsheets and various databases) and report writing, an understanding of commissioning processes, and will be able to identify and address operational gaps in service delivery in the context of youth housing. You will also have an excellent understanding of the challenges, needs and risks that affect young people in and leaving care and will act as an advocate for young people leaving care, working in a tenacious, creative and dynamic way to find bespoke solutions to meet their housing needs. Working with P3, Probation Service, Police, Housing and Benefit Services you are required to provide advice and guidance to customers to assist them with access to general needs housing and their successful reintegration back into the community. About the Role Southend Medical Centre are looking for a motivated individual who has a passion on tackling health inequalities, and particularly an interest in healthcare for the homeless population. We are seeking an experienced prescribing paramedic or nurse on an 11 month maternity contract, commencing in September, on a part-time basis for approximately 22.5 hours a week, working over 3 days. Hours and working pattern can be negotiable for a suitable candidate. Reporting to a GP clinical lead, previous experience of working with this population group is desired but not essential. Southend Integrated Healthcare for the Homeless (SIHH) provides a service specific to homeless individuals within Southend on Sea. The role will include a range of activities addressing the individual health and wellbeing, the development of pathways into specialist services, caring for the homeless and improve the overall health inequalities by: <ul> <li>Health promotion</li> <li>General health screening</li> <li>Engagement in primary care services and facilitate access to organised health care</li> <li>Integration of the service with secondary care services, mental health services and local drug and alcohol services</li> <li>Working closely with local voluntary providers involved in looking after the local homeless population.</li></ul>The purpose of this role will be imperative in improving integrated pathways to prevent fragmented services to become efficient and effective.
