Job summary

An exciting new opportunity has arisen to join the Stop Smoking Service team based at Forsyth House, supporting the delivery of a high-quality smoking cessation service across Darlington.

This role will play a key part in enhancing and maximising opportunities to provide Very Brief Advice (VBA) to people who smoke across all primary healthcare settings. By working collaboratively with healthcare professionals and community partners, the postholder will help increase engagement with stop smoking support and contribute to reducing smoking prevalence locally.

Main duties of the job

to support the Stop Smoking Service team, based at Forsyth House, Darlington, in delivering a smoking cessation service to the local population. The role will enhance and maximise opportunities to deliver Very Brief Advice (VBA) to people who smoke across all primary healthcare settings. This will contribute to key outcomes of the service and necessary support to reduce smoking prevalence in Darlington, with a particular focus on targeting and engaging those most likely to benefit from stop smoking assistance. The role will involve some evening and weekend working, promoting the service within the community in and around Darlington.

Primary Healthcare Darlington is a GP Federation, based centrally in Darlington, which supports 11 GP practices in Darlington to provide the best possible healthcare services.

We also provided an extended GP Access Service, a Community Sexual Health Service, private medicals and primary care services and an integrated service to the Care Homes within Darlington.

Tobe the key liaison between the Stop Smoking Service and the GP Practices, aswell as other primary health care settings, acting as a Single Point of Contact(SPOC) for any queries or questions and provide additional support.obe able to fully explain how the Service operates, including how to refer tothe service and what stop smoking support aids are available to support themThis includes undertaking relevant training on NRT, vapes and Pharmacotherapy

Supporting clients within Darlington to refer to thestop smoking service or begin the behaviour change journey

Providingcontinuous support to people to make the decision to make a quit attempt or totry again.

ProvideMini awareness sessions/drop ins from any primary health care setting waitingareas, as appropriate, promoting the Service and outlining the productsavailable to support smokers quit and offer links to relapse prevention.

Workingwith Primary Care colleagues the champion will target their support to peopleliving within the borough, with long term conditions exacerbated by smoking.

Beresponsible for requesting and coordinating the supply of medication and/orquit aids

Have access to and be competent in using System1 tobe able to target areas of borough with the highest smoking prevalence tosupport them to refer into the service.

Participatein community engagement activities across Darlington to promote smokingcessation services

Administration

To have a thorough knowledge of all department procedures.

To work in accordance with written protocols

Recording information within the client's medical records(SystmOne) and other data collecting systems

Ensuring messages and information are passed on to relevantmembers of staff or outside agencies

Running reports

Telephone

Have working knowledge of the telephone system, during and afterhours.

Provide an efficient system of answering the telephone using acourteous and friendly manner

Liaising with healthcare professionals and other professionalbodies

Other Tasks

Attending community events and promoting the service

Any other tasks allocated by the service team Management

Promote MECC(Making Every Contact Count) by making the most of an opportunity to raiseawareness, share knowledge and get a person thinking about making changes toimprove their health, wellbeing and behaviours.

Deliver VBA and /orinterventions, that can be 'opportunistic', taking very little time and can beimplemented anywhere on the intervention continuum.

Deliver brief interventionsrecognising that many people can benefit from being given appropriateinformation at the right time particularly relating to the harms and risksassociated with tobacco and smoking and other risks such as second-hand smoke.

Provide support to stopsmoking through encouragement and behaviouralchange support

Run data reports and requestson system one
