An exciting new opportunity has arisen within the Homeless Health Team to help support them administratively. This role is a new role that has been funded as a 1 year fixed term contract to focus on improving patient outcomes and experience, alongside reducing their frequency of needing to access hospital care, by front loading support at initial contact to ensure a safe transition into the community.

This team is focused heavily on improving patient outcomes which is monitored regularly for patients with no fixed abode, or who are at risk of homelessness as they can often face significant delays in the acute setting. This team supports at the earliest opportunity in their admission to reduce delays and the impact.

This role will be integrated within the discharge team administration function, to provide a robust administrative service across Cross-site, improving resilience across the administrative function for all discharge pathways.

We are looking for a highly motivated individual who will perform their role in a calm and professional manner, communicating effectively with hospital staff and external colleagues.This is a role that requires sensitivity and proactiveness, so will be well suited to an individual who is approachable, confident, with strong organisational, interpersonal and communication skills.

Main duties of the job

  • To ensure quality care and support service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety.
  • To support the administration of the Homeless Health Team to improve patient outcomes.
  • To provide administrative support to the Cross-site Discharge team
  • To work in support of qualified staff and allied health care professionals in the assessment, planning, implementing and evaluation of care without direct supervision of a registered practitioner
  • To always communicate effectively within the team, other health care professionals and with the patient family, carers/ visitors
  • Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.
  • To regularly assess evaluate and report on actions and activities to registered practitioner and/or Multidisciplinary Care Team.
  • Collaborate with members of multi-disciplinary, multi-agency team to ensure that patients individual needs, autonomy and independence are promoted.
  • Lead on administrative tasks, such as patient transport booking, ordering equipment, maintaining patient activity log for senior staff to carry out audit activities.
