Job summary
We're looking foran experienced practitioner with a strong sense of social justice to join our HomelessHealth Inclusion Team. We delivertransformational models of holistic healthcare to people experiencinghomelessness, the Gypsy and Traveler community, and other inclusion groups inLeeds experiencing homelessness via outreach and in-reach programmes inpartnership with the third sector, community members, Primary Care, and LeedsTeaching Hospitals Trust. We advocatefor our patients and plan safe discharge, providing out of hospital care forour patients and lead the health care for patients in Transitional HousingUnits and beds at St Georges Crypt. You will case manage and work with patientson their health goals to ensure health needs are met and a reduction inemergency hospital admissions. We provide an outreach function to the Gypsy andTraveller community, working with Leeds Gypsy and Traveller Exchange and facilitatinghealth equality for their members. Developingcurrent links with the street teams, you will work closely with teams whoprovide street outreach, signposting and building relationships with those whoare rough sleeping and those new to Leeds.Working with those identified as suffering from homelessness and thoseidentified as sofa surfing; vulnerably housed; victims of trafficking; exploitation;vulnerable women fleeing domestic violence; refugees; those in the asylumprocess; failed asylum seekers; those with no recourse to public funding.
Main duties of the job
You could be acommunity nurse, health visitor, OT or mental health nurse. You will need tohave a passion for ensuring equality of health for all. You're flexible and ableto work under pressure and in a challenging and fast-paced environment. As you'll be working with people who haveexperienced complex trauma highly effective communication skills are a must. Youare empowered by working independently but able to maintain a sense of team andyour professional identity.

- Providingholistic clinical assessments and care.
- Coordinatingcomplex safe discharge planning from hospital
- Identifyingand implementing appropriate interventions
- Providingeducation to stakeholders
- Jointworkwith Forward Leeds in reach team and Forward Leeds HubincludingBevanHealth Practice, around drug/alcohol use andtreatment
- Signpostingto Primary Care
- Continuingclose links with Bevan Healthcare (homeless and vulnerable Primary care healthcentre in Leeds)
- Multi-agencyworking with all partners in Leeds who work with marginalised groups such asprobation, sex working projects, Leeds Gate, Leeds Housing Options, BeaconEmergency Housing, St Anne's Day Centre, Adult Safeguarding, Adult Social Careetc
- Attendingcity wide meetings such as the High Intensity of ED group, Risk assessmentmeetings, Multi agency meetings.
