Job summary
A year fixed opportunity to lead the Homeless Health Team at London Northwest Healthcare trust.
You will provide clinical expertise and knowledge and will provide daily leadership and operationally manage the team.
The Homeless Health Team works in close partnership with the wards to identify people experiencing homelessness whilst they are in hospital, build therapeutic relationships with them, assist the ward multidisciplinary team to maximise the benefit of these admissions, and then support these vulnerable patients during and after discharge - ensuring that they are fully linked in to all the health care and support services they need when the team finally discharges them.
Main duties of the job
To provide overall day to day operational leadership and direction to the Homeless Health Hospital Team across the hospital.
To facilitate and maintain active positive links with all key partnership agencies (e.g., homeless health services, housing, social care services, and the voluntary sector) always, developing projects and protocols to improve working relationships and service delivery as necessary.
To develop excellent links and working relationships with the Integrated Discharge Hubs and clinical teams within the hospital maximising benefit to patients.
To be an expert in relation to the physical health, mental health and addiction related conditions that commonly effect homeless people and deliver advice across the hospital as necessary.
To have expert knowledge in the physical health, mental health and addiction related conditions that commonly effect homeless people and apply this to the care of own clients.
To act as a resource for other staff within the Trust, providing direct support for other professionals around complex discharges as necessary. In line with this to have expert knowledge on the possible discharge solutions available for homeless people.
