Key Focus
To provide individual and bespoke support to young people/adults at risk of homelessness and signpost to appropriate services to meet the clients needs.
You will help to prevent homelessness and support people to create self-determined outcomes that will lead to sustained positive change through the creation of support plans, innovative interventions and a coaching style approach.
(The role is part of the wider Pathfinder work across Greater Manchester)

Key Deliverables
- Deliver wrap-around, trauma informed support to young people within the community and their own independent tenancies
- Prepare and develop individually tailored support plans, achieve outcomes in accordance with contractual Key Performance Indicators
- Perform robust assessments of referrals for the service to ensure the service is provided to those that meet the eligibility criteria
- Maintain accurate, up to date records on the case management system against individual support plans, safety plans and outcomes information
- Liaise with local specialist services to facilitate access to support including: locally based offers, Criminal Justice services, health services, employment and training agencies
- Ensure that individuals have an understanding of financial management in preparation for independent living, including focus on rent payment and budget creation
- Build relationships with landlords, housing agencies and local housing teams
- Complete dynamic risk assessments and maintain own safety by following lone working procedures
- Adhere to good practice Safeguarding procedures to ensure the wellbeing of vulnerable people.
- Long Service Leave: an additional 4 weeks paid holiday after every 3 years in service (subject to attendance thresholds).
- Supervision: 10 sessions per 12 months with an external supervisor to facilitate reflective practice.
- Wellbeing Day: 2 x half days of additional paid leave every 12 months as a 'wellbeing day' for staff.
- Volunteer Day: 1 day additional paid leave every 12 months as a 'volunteer' opportunity with another third sector organisation of your choice.
