We are looking to appoint a Homelessness Prevention Officer to join our Young People's Pathway Team.

This is an exciting opportunity to work with young people aged 16-25 who are at risk of homelessness or homeless in North Yorkshire . B ased within the Housing Options team the post holder will be required to work alongside colleagues in Children and Families Service, Adults Social Care and a variety of external agencies all of whom support the Young People of North Yorkshire.

The post holder will work with young people and their carers to prevent homelessness through timely assessment and appropriate intervention , including mediation . The role is a hybrid one that provides linkage between housing and children's social care, and the post holder will receive training in this role .

The pos t holder will work closely with Housing Options Officers where there are statutory duties to the young people around accommodation and Children's services when young people are aged 16 and 17 or leaving care . Through joint working, this post will support the effective delivery of the Young People's Accommodation Pathway.

Duties include:
  • To manage a case load of young people at risk of becoming homeless, including assessment, planning and administrative duties.
  • Ensure that screening assessments for all 16 and 17 year olds are completed and recorded within the appropriate databases.
  • To assess and monitor risk levels and develop appropriate support plans for young people, referring to specialist support services as required and monitoring and co-ordinating the implementation of plans.
This is a hybrid role primarily covering the towns and rural areas around Malton, Helmsley and Pickering, with a day per week (sometimes more) based in Northallerton. The post holder will support the team across North Yorkshire as and when required to meet the needs of the service.
