About the role

This is an exciting time to be part of the Housing Options Service and the Council's major new Homelessness Transformation Programme. Join us in bridging the gap in healthcare and Housing.

We are seeking a dedicated and compassionate Hospital Discharge Housing Coordinator to join our team. In this vital role, you will play a key part in facilitating safe and supported transitions for patients in the hospital who may be threatened with homelessness or are homelessness. Your work will directly impact individuals' well-being and contribute to reducing homelessness among vulnerable residents of Tower Hamlets.

As a Hospital Discharge Housing Coordinator, you will work closely with healthcare professionals, social workers, housing providers, and patients to ensure seamless transitions from hospital settings to stable housing and work with the patient to prevent homeless, where possible. You will conduct homelessness assessments and the housing needs of applicants supporting them to develop a personalised housing plan and issuing statutory notification letters individuals' housing needs. Coordinating temporary accommodation placement arrangements and provide ongoing support to promote successful reintegration into the community.

The Person

We are looking for a candidate who have knowledge of the Housing Act, housing options, support services, and local housing policies, particularly related to homelessness. Previous experience in housing support, social care, or healthcare coordination, with a specific focus on homelessness prevention and intervention. If you have excellent interpersonal and communication skills and can engage effectively with stakeholders and work collaboratively within multidisciplinary teams to build positive relationships, we are looking for you. If you have strong problem-solving abilities to assess complex needs and develop tailored housing solutions with exceptional organisational skills to manage caseloads and coordinate discharge processes efficiently. If you are confident, experienced, and are an adaptable self-starter with the ability to work proactively on your own initiative, then you may be the one we are looking for
