Job summary

Southern Health NHS Foundation Trust provides community health, mental health, learning disability and social care services for people across Hampshire and its surrounding area. The Trust aims to provide high quality, safe services which improve the health, well-being and independence of the people it serves.

As part of the Trust Acute & Crisis Division, you will be a key member of the patient pathways team providing excellent mental health care to people in the local area. You will be leading and influencing change during an exciting time of transformation for the specialty.

This post is based at Antelope House, Mental Health Unit in Southampton.

You will be working as part of a multidisciplinary team to provide support to our homeless clients or clients needing support with their housing and social care needs. In addition, you will help contribute to a great and collaborative working environment and be a crucial member of the Patient Pathway and Bed Management Team within the division.

Main duties of the job

*You will have a list of clients to manage and work with them on a one to one basis.

*You will need to identify barriers to discharge whilst admitted as an inpatient in our inpatient units at the earliest possible opportunity and work proactively with the multidisciplinary team to unblock these discharges and support the team.

*You will identify their social care needs by listening to them and being part of the wider multidisciplinary team. You will do these by attending MDT meetings, ward rounds, one to one meetings with clients.

*You will support to move clients on from inpatient units to suitable and identified longer term accommodation options.

*You will mediate and negotiate and develop good and positive working relationships with existing housing providers and external providers to help prevent homelessness.

*You will help and support individuals by providing, engaging complimentary partners who can provide advice on housing, welfare benefits, rent deposits and household goods by accessing a variety of schemes.

*You will signpost individuals, when appropriate. These will include signposting to services such as debt management, housing issues etc

*You are expected to undertake other reasonable duties require in the interest of the business.

*You will need to have a good understanding of discharge flow process in inpatient units.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
