Job summary
To work in a clinical capacity providing mental health interventions - including assessments and treatment to clients at risk of experiencing homelessness or with a history of experiencing homelessness, as identified by Test Valley Borough Council's Housing Options Team, or Hampshire and IOW NHS Healthcare NHS Foundation Trust's community mental health or the inpatient flow teams outside of established pathways.
To contribute to breaking down the barriers to accessing and sustaining health care and suitable settled accommodation for the identified client group. To actively work with other agencies to provide wider support and address underlying causes of homelessness, developing referral routes between the Council and partner organisations across the wider system (including statutory and voluntary sector services).
This is a 12 month fixed term contract.
Main duties of the job
To work with single people who are at risk of or have a history of, experiencing homelessness and or rough sleeping known as the client group in various settings.
To identify and assess mental health needs of the client group, outside of the normal established mental health provisions and pathways.
Undertake comprehensive assessments, over a period of time, including risk assessments and risk management of the client group.
Utilise advanced clinical skills in assessment to formulate a treatment plan, including monitoring the response to medication and assess critically and re-evaluate the clinical situation as the patient's condition changes, reporting back to the prescriber as appropriate.
To support patients of the client group to make changes to support recovery from mental health illness, substance misuse and maladaptive coping strategies.
To take the lead in multi-agency meetings in support of those affected by poor mental health. This includes arranging and chairing multi-agency professional and risk management meetings.
To raise safeguarding concerns where appropriate
To ensure confidentiality and data protection are observed at all times in compliance with the Data Protection Act.

- Registered practitioner (mental health nurse, social worker, occupational therapist or equivalent) and registered with the relevant professional body
- Diploma or degree level qualification in mental health
- Must hold a valid UK driver's license and have access to a vehicle for work use. .
- Experience of working with clients/patients who have experienced homelessness or rough sleeping

- Experience of working with service users who present in crisis and can demonstrate sound knowledge of safety planning and risk management strategies and approaches.
