Theaim of the role is to help equip people to sustain tenancies in the communityand their long term recovery from substance misuse.
Main duties of the job
Working with a case load of clients
Completing assessments
Creating support plans and risk management plans
Recording accurate client information via a case management system
One to one and some group work with clients
Signposting to community services for wellbeing, training, education, benefitsand housing
Signpost to community recovery support groups
Supporting clients with move on process
Carrying out H&S checks within our properties
Supporting clients to increase independent living skills
Supporting clients to understand how to maintain future tenancies
Dealing with Anti-social behaviour
