The role includes lone working, travel between properties, and participation in an on-call rota.

Key Responsibilities:
Support Delivery
- Deliver one-to-one support to service users based on assessed needs, risk level, and support plans
- Complete and regularly review support plans
- Build supportive, professional relationships that promote trust, independence and accountability
- Help clients engage with community services, manage their tenancy, and sustain accommodation
- Support clients with areas such as mental health, substance use, offending behaviour, and self-care
- Monitor and manage client risk in line with safeguarding and lone working procedures
- Respond to crisis situations, including critical incidents and welfare checks
- Escalate safeguarding concerns promptly to the Operations Manager or Designated Safeguarding Lead
- Work in partnership with Probation, MAPPA, housing, and other statutory services
- Attend and contribute to multi-agency meetings when required
- Record all support sessions, risks, incidents and contact notes promptly and accurately
- Comply with data protection, safeguarding, health and safety and Axis policies at all times
- Assist with audits, reviews, and evidence gathering for service improvement and reporting
- Maintain confidentiality and uphold professional boundaries at all times
- Work closely with the Operations Manager and other staff to ensure safe and effective service delivery
- Follow all lone working and on-call procedures to ensure safety
- Participate in training, reflective practice and team meetings
- Uphold Axis's values of safety, dignity, inclusion and professionalism
- Service users
- Operations Manager and Senior Management Team
- Probation Officers, MAPPA, mental health teams, housing officers
- Landlords, contractors and local authority services (where needed)
- Complete all new client support plans within 10 working days
- Maintain 90%+ completion rate for case recording within 24 hours
- Participate in 100% of required lone working check-ins and on-call duties
- Support at least 70% of clients to achieve one or more agreed outcomes within 6 months
- Complete all mandatory training within 3 months of starting
Essential:
- Experience supporting people with complex needs or risks
- Understanding of safeguarding, lone working, and client confidentiality
- Ability to stay calm and manage challenging situations independently
- Strong written and verbal communication skills
- Ability to travel independently between dispersed locations
- Experience of working with criminal justice-involved adults
- Familiarity with risk assessments and psychologically informed practice
- Experience of working with trauma, substance use, or homelessness
- Enhanced DBS check
- Lone working across multiple properties
- On-call duties on a rota basis
- Full UK driving licence and access to a car
- Salary: £28,392 per annum
- Hours: 37.5 per week. Hybrid working as suits the needs of the business and in agreement with the manager.
