Calderdale Inclusion Support Service provides Mental Health Support in collaboration with South West Yorkshire NHS Trust. Within this service we are recruiting to the role of Housing Support Worker to assist Calderdale adults with mental health needs access and maintain suitable housing. Our objective is to offer timely advice and support to enable individuals to acquire, set up and retain secure tenancies. Our referrals come from the mental health services namely the Community Mental Health Teams, rehabilitation units and acute wards, as well as Calderdale Council Social Care Teams. We work closely with agencies to maximise choice and promote independence.

You will assess and identify housing needs, and develop and implement housing support plans, in consultation with the individual and keeping them at the centre of the process. You will need to plan for and arrange meetings and ensure case records are accurate and up to date. In addition, you will need to be able to use IT packages and systems independently. Moving home can be a stressful life event and you will need to understand different coping mechanisms, offer support and liaise with the wider support team, to promote the individuals' mental well-being.

You will be work in a hybrid model working from Council, and NHS offices as well as being able to work from home. You will undertake visits to see clients at local hospitals, residential schemes, clinics and clients' homes. As such you will need to be able to work independently to meet the needs of the service.

You must hold a full UK driving licence and have access to a car.

If you have experience of working with adults with mental health needs and multi-agency working, and a knowledge of housing we would welcome your application. The successful applicant will have a strong person-centred value base, empathy and effective interpersonal skills. You will be able to work on your own initiative, problem-solve and be able to work effectively as part of a team. A qualification of Level 3 Health and Social Care or equivalent, or demonstrable experience is required.
