The Role

Our Independent Living Advisors provide high quality information, advice and support to both referring Health & Social Care professionals and vulnerable eligible customers who are considering participating in Self Directed Support Schemes, Independent Living Schemes, Personal Health Budgets and self-funded/private clients, over a designated area. We are looking for individuals with a background in social care who can support children and adults with disabilities to live independently in their local communities. They work remotely and arrange home visits with their customers in order to make sure they have all the support they need to retain their independence.

This role is a remote working role, with occasional community engagement around the Brighton and Hove area. The successful candidate will be working remotely, and engagement with our teams, clients and partners will be mostly via email/phone/Teams. This position is fixed term for 6 months.

What does the role involve?
• Providing a person-centred service that enables individuals to focus on the outcomes they wish to achieve; supporting with establishing a care and/or support plan that helps the person we support meet these outcomes in a cost-effective and safe manner
• Managing a caseload within a geographic area, including visits to people we support's homes, local authority offices and other community locations in Brighton and Hove.
• Assisting people we support to manage their obligations and responsibilities in relation to their chosen care package
• Obtaining and collating information for caseload management and reports on a daily basis, utilising chosen PeoplePlus and/or designated databases
• Maintaining accurate records of all communication with people we support utilising chosen PeoplePlus and/or designated databases
• Ensuring all information given is clear and concise and in a format that meets the individual communication needs of the person you are supporting.
• Ensuring all information is factually correct and in accordance with PeoplePlus guidelines and current legislation
• Organising and attending meetings and/or reviews with people we support, colleagues and other professionals as required within Brighton and Hove.
