About the Program:
Insync is a program for young people who are at risk of, or who are currently experiencing, homelessness in Brisbane, Redlands and Logan. Our team supports young people with crisis and transitional accommodation, while helping to increase their living skills and achieve their goals through a case planned process.

About the role:
The overall purpose of the Practitioner is to provide case management, care and support to young people living in our accommodation or in the community. The care and support provided will enhance opportunities for the young person to participate in appropriate activities to support development community inclusion, positive relations with family of origin and to contribute to their accommodation stability.

This position is a 5 Month Fixed- Term Contract, working Part-Time, 24 hours per week at Insync Cleveland, providing leave coverage until 10th February 2025. Specific hours/times can be negotiated.

Responsibilities:
  • Provide comprehensive case management, mediation, and advocacy for young people and their families, aligning with Anglicare's practice framework.
  • Facilitate goal-setting sessions, assess needs, and coordinate access to community and government resources to support youth development and family stability.
  • Develop living skills programs and collaborate with specialist services to ensure integrated service delivery within a case management framework.
  • Contribute to the review and development of the organizational business plan, identifying service gaps and participating in community project development.
  • Maintain meticulous documentation and reports, ensuring compliance with organizational standards and procedures.
  • Assist in the development and review of policies and procedures, and update resources from external agencies to support effective service delivery.
  • Foster a positive team environment through active participation in staff meetings, providing peer support, and contributing innovative ideas for continuous improvement.
  • Pursue ongoing professional development opportunities to enhance skills and stay abreast of industry best practices.
Qualifications, Credentials and Other Requirements:
  • Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar (this is an essential requirement of the role)
  • Current National Police Check
  • Current Blue Card
  • Current Queensland Driver Licence
  • Ability and willingness to travel within the service region
