Join our dedicated Joint Homelessness Team in Central London! We are a forward-thinking local authority committed to supporting adults experiencing mental illness and homelessness, including those with No Recourse to Public Funds (NRPF). As part of our team, you will play a crucial role in delivering high-quality Adult Social Care (ASC) functions to some of the most vulnerable members of our community.
Position Overview:

We are seeking an experienced Mental Health Social Worker with a strong background in Adult Social Care (ASC). The ideal candidate will be skilled in working with adults, including older adults, facing mental health challenges and homelessness. Your role will encompass a variety of responsibilities, including casework, placement reviews, and other key ASC functions.
Key Responsibilities:

Assessment and Planning:
  • Conduct comprehensive needs and risk assessments under the Care Act and other relevant legislation.
  • Collaborate with service users to establish eligibility and develop personalized care and support plans, including crisis support plans and personal budgets.
  • Participate actively in reviewing and updating care and support plans.
Carers Assessments:
  • Identify carers and perform statutory Carers Assessments under the Care Act.
  • Regularly review Carers Support Plans in compliance with statutory requirements.
Safeguarding:
  • Identify safeguarding issues for adults and children, implementing appropriate procedures.
  • Execute key duties under safeguarding procedures, potentially acting as Lead Enquiry Officer.
Record Keeping:
  • Maintain accurate and up-to-date records of all work in accordance with Local Authority policies and procedures.
  • Ensure all data is accurately entered into the relevant IT systems of the Local Authority.
Supervision and Development:
  • Utilize supervision sessions for effective workload management and professional development.
  • Stay updated on relevant legislation, government, and departmental policies and procedures.
  • Participate in local social work forums and multidisciplinary team meetings as outlined in the Memorandum of Understanding.
Communication and Collaboration:
  • Communicate effectively, both in writing and verbally, with service users, carers, and other professionals.
  • Establish and maintain productive working relationships with multidisciplinary teams and external agencies.
Work Environment and Professionalism:
  • Perform effectively in potentially stressful environments, prioritizing workload efficiently.
  • Reflect on and evaluate personal practice, accepting and acting on constructive feedback.
  • Demonstrate flexibility in working practices, occasionally extending working hours if required.
  • Treat service users with respect and dignity, adopting a culturally sensitive approach.
  • Utilize high-standard IT skills for accurate record-keeping on the borough's electronic database.
Requirements:
  • Social Work England Qualification: Essential
  • Extensive experience in adult social care, particularly with mental health and homelessness.
  • Strong understanding of the Care Act and related legislation.
  • Excellent communication, assessment, and planning skills.
  • Ability to work effectively in a multidisciplinary team.
  • Commitment to safeguarding and promoting the welfare of vulnerable adults.
