RESPONSIBLE FOR: Undertaking specialist assessment and care coordination of individuals presenting with housing problems, including use of substances Delivering interventions for substance use in both group and individual formats, also holding a caseload of service users Providing consultation, support and advice in relation to substance use issues Holding a caseload of service users, being responsible for offering a range of interventions to those individuals. Including harm reduction interventions Ensuring that your work is evidence based, up to date and in accordance with current policy and legislation. Keeping up to date and accurate records and documentation.
Main duties of the job
JOB PURPOSE
To provide a dedicated, comprehensive service to people experiencing homelessness, being at risk of homelessness and having substance related problems or being at risk of relapse back into substance use and have a history of, or are at risk of disengaging from services. To also work closely with and provide advice and guidance to colleagues within the housing options team to integrate substance use intervention into care plans, increasing engagement and ongoing support, where appropriate.

- To offer Naloxone to all Clients due to risks presenting, or as a precautionary measure.
- To facilitate access to BBV testing
- Be responsible for maintaining a full case load of clients as the case manager.
- To work with the housing options team and other colleagues to reduce stigma for people who use drugs and alcohol. And to reduce stigma for people with homelessness complexities to enable access to care and support in the community
