An exciting opportunity has arisen for suitably qualified and experienced range of staff to join the Camden Adult Pathway Partnership (CAPP). This service is a joint collaboration between UCLH and the Borough of Camden providing health clinics in Camden hostels.

We are seeking highly committed and motivated individuals who will ensure that we provide an effective wraparound healthcare service for homeless people within the Camden Hostel Pathway. The post is required to work as a team with the Head of Integrated Operations focussed upon developing effective collaborations with general and specialist clinical teams, hostel managers and staff and other outreach homeless services across the integrated care system internally and externally to UCLH, to provide the healthcare to the patients living within the Camden hostel pathway.

This vacancy has been advertised in accordance with the new NHS pay rate which will take effect from autumn 2024. Please note if your employment starts before the 24/25 pay scales are implemented you will be paid under the 23/24 pay scales and any backpay will be adjusted accordingly. Main duties of the job

We are looking for an experienced nurse with the ability to work in an innovative and flexible manner with the CNS and clinical project lead to ensure that an effective healthcare service is provided.

This post is fixed term until 31st March 2025, with a potential to extend pending confirmation of further funding.

Knowledge and Qualifications

Essential criteria
  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • Registered Health Care Professional e.g. NMC.
  • Knowledge of the particular issues that characterise the provision of health services to vulnerable groups and homeless persons.
  • Evidence of continuous personal and professional development and willingness to commit to ongoing CPD.
  • Further training or experience in a clinical supervisory position.
Experience

Essential criteria
  • Previous significant up to date experience within the acute care setting is required.
  • Clinical Experience required Including; wound care management, Infection control, Chronic illness support, Safeguarding and Protection, Medication and Administration, Protection of vulnerable adults, phlebotomy, minor injuries, Care planning
  • Experience of working in community setting/general practice
  • Demonstrate ability to lead a clinical shift or clinic
  • Evidence of active participation in and/or experience in teaching or delivery training
  • Strong political awareness, and evidence of working well within highly sensitive environments
Skills and Abilities

Essential criteria
  • Ability to analyse and interpret information, pre-empt and evaluate issues, and recommend and appropriate course of action to address the issues.
  • Problem solving skills and ability to respond to sudden unexpected demands.
Desirable criteria
  • Familiarity with electronic patient database, i.e. EPIC.
Communication

Essential criteria
  • Skills for communication on complex matters and difficult situations, requiring persuasion and influence
