Job Introduction

An exciting opportunity to lead a team of skilled Social Workers and work jointly with partner agencies to support those who are homeless.

The Homeless Intervention Team have the following criteria -
  • Aged 18 or over
  • Requiring single or couple occupancy accommodation
  • Appear to have care and/or support needs which would benefit from further assessment or intervention from ASC to contribute to sustainable accommodation or relief of homelessness
  • Not currently open to another Adult Social Care (ASC) team or Community Mental Health Team (CMHT)
  • Open to the Housing Team with an active homelessness relief or prevention duty (Housing Act 1996), or are a verified rough sleeper (with or without an open duty under the Housing Act 1996)
The team sit within Statutory Services and under the Safeguarding Adults Statutory Service. The team work alongside partner agencies to undertake S42 Enquiries and Care Act Assessments.

The role of the team manager is -

MAIN PURPOSE
  • To work in partnership with the Service Manager to ensure effective leadership and management of multi-disciplinary staff. This will entail responsibilities such as recruitment, supervision of staff, allocation of resources and case work.
  • To supervise and provide professional advice and guidance to staff.
  • Be accountable for statutory assessments and their outcomes.
  • Undertake assessments of a complicated nature that require multi-disciplinary working, application of policy, procedure and law.
  • To act as a catalyst for ongoing changes in service design and delivery for adults and older people across a health and social care spectrum within the locality.
  • To be responsible for specialist budgets, for example, Community Equipment Budget.
MAIN RESPONSIBILITIES

To ensure and contribute to the:
  • Promotion of person centred working and active engagement of adults and older people in developing independence, choice and control in the design and delivery of their care.
  • Proactive working with the patients/service users, providers, GP's and health trust staff.
  • Application of advanced skills in identifying changes in client's conditions and take action to prevent deterioration.
  • Full awareness and application of Adult Safeguarding and mental health policies and procedures.
  • To deputize for the Service Manager as required and to ensure the continuity and efficient delivery of service by the team and to provide cover for the other Team Managers in their absence
  • To practice as a BIA as required
  • To contribute to the maintenance, development and improvement of services to promote evidence based good practice. To attend meetings, contract meetings and workshops as directed and lead on the development of specific projects
