Job summary An exciting opportunity has arisen for a hub manager for the BSMHFT Community Mental Health Team for Homeless People, Homeless Health Exchange Primary Care Team and the NHSE funded Mental Health Outreach Team for Rough Sleepers. All teams operate city wide to meet the needs of Homeless adults with enduring mental health problems and often substance misuse issues who require significant engagement due to their chaotic lifestyle. The teams work in partnership with other homeless commissioned statutory and third sector organisations for accommodation, housing and tenancy support, physical health care and substance use services. The CMHT is a small team whose CPN's work with a consultant psychiatrist to provide secondary care level mental health interventions with a focus on stabilisation of lifestyle, recovery and moving out of homelessness.
