Job Introduction At Turning Point, we support people across the UK with mental health issues. As the Transition Navigator, you'll make a real difference to their lives as you work closely with them to help them achieve their potential. Beacon Lodge is a mental health step down unit that supports people to transition from acute mental health wards back into the community. Passionate about people, you'll enjoy the scope of this role. With an emphasis on recovery working alongside specialist housing and benefit related support, you'll need to work flexibly to manage your own small caseload and help with the day to day running of the service. Role Responsibility Raising the bar for person-centred care, you'll provide a range of recovery focused support through key working sessions and group work. People's lives will be truly changed by you as you enable and prepare them to gain new skills and help them find ways to promote their long-term recovery. You will also have off rota responsibilities such as housing and benefit assessments, identifying housing needs, liaising with local authorities to ensure that service users are supported in their move on from the service. On rota, you'll work with individuals on their recovery, devising individually tailored support and risk management plans and ensuring effective discharge planning. Ultimately, you'll play a vital role in helping them to gain skills and coping strategies, all while encouraging positive change and independence. The role requires you to work on a 24-hour rolling rota consisting of 12-hour day and waking night shifts including alternate weekends. The Ideal Candidate Ideally with experience in the mental health sector, it's essential that you have a good understanding of the recovery model, together with a solution-focused attitude and a person-centred approach. You'll be equally comfortable getting things done on your own initiative and as part of a team and have no problems working in a fast-paced environment and adapting to challenging situations. And of course, you'll be the sort of person who enjoys talking to people with different needs, is genuinely interested in helping them, and possesses a deep level of empathy, understanding and patience. Housing and benefit knowledge would be desirable in this role but support and training will be provided to the successful candidate. Transition and Housing Support Worker

Job Introduction Bradford Cyrenians provides a range of supported housing for vulnerable communities, our services are flexible and user-led, with a focus on prevention, enabling individuals to take control of their lives. We are recruiting for a Supported Housing Officer for our Hostel/Homes4Life service. This is an exciting opportunity for someone who is passionate about providing the best possible support to our service users Working flexibly between the hours of 8.00am to 8:00pm Monday to Friday. Shift work on a rota is a requirement of the role. May be required to work weekends or other shifts in an emergency. Salary - £23500. The candidate needs to be highly motivated and have the passion, enthusiasm and commitment towards making a difference to the lives of vulnerable communities.
